Fixes the organisation profile.

• brew install terraphim-ai does not exist: now terraphim-grep and terraphim-agent, both present in the tap.
• cargo install terraphim-agent is the wrong crate name: now terraphim_agent, plus terraphim_grep.
• vscode-terraphim and terraphim-wxt-extension return 404: now link the extensions in terraphim-ai.
• terraphim-skills is private, so its stars badge cannot render: now links the public https://terraphim-skills.md site.

Checked: both linked files exist on terraphim-ai main, both formula files return 200, and both crates exist on crates.io. The install commands themselves were not run from this PR.
